Vaccine Exemption Procedure for Clinical Placements
There are several clinical placements that may require students to have specific vaccinations. If you are unable to receive certain vaccines due to religious or medical reasons, please read and complete the following procedure.
This procedure applies only to students enrolled in clinical placements where vaccination is required. If you are a new student and have not already completed 老虎机攻略 admissions vaccine requirements, review our New Students Immunization Requirements.
Please ensure that all required forms are completed accurately and submitted promptly to avoid any delays in your clinical placement.
Types of Exemptions

- Students should download the Medical Exemptions for Vaccines Form and have it completed by a licensed healthcare provider
- The narrative should specify the medical reason for the exemption, and, if possible, the expected duration of the contraindication
- Once the form is completed, email it to the Office for Disability Equity (ODE) at ode@umontana-edu with the subject line "Clinical Vaccine Exemption Request"
- The Office for Disability Equity will contact you to confirm receipt of the form. If the form is complete and the exemption is granted, you will be notified and provided with the appropriate documentation to give to your clinical placement or the relevant person in your academic department
- In rare cases where a medical exemption cannot be granted but a medical condition exists, possible accommodations can be discussed

- Students should download and complete the Religious Accommodation Form
- The narrative should include a written statement explaining the religious believes that prohibit vaccination
- Once the form is completed, it can be sent to the Dean of Students Office at deanofstudents@umontana.edu with the subject line "Vaccine Exemption Request"
- The Dean of Students Office will contact you to confirm receipt of the form. If the form is complete and the exemption is granted, you will be notified and provided with the appropriate documentation to give to your clinical placement of the relevant person in your academic department
Semester Withdrawal and Mitigating Circumstances Policies
All withdrawals are currently processed by the 老虎机攻略 Registrar's Office. Students who have experienced catastrophic, life-altering events that prohibit the student from completing the semester may file for a mitigating circumstances appeal once they have completed the from the university.
