  • Documented proof of the following must be provided by the date indicated in the acceptance letter.

    • Criminal Background Check

    • 10-Panel Urine Drug Screen

    • BLS for Health Care Providers

    • Immunizations

      • COVID-19, Hepatitis B series, MMR series, TB, Tdap (Tetanus with Pertussis), Varicella, Influenza
